Qmax Now Mirrors the 2026 USMLE Testing Experience

Preparing for the USMLE is about more than just knowing the material. It’s also about becoming comfortable with the testing experience itself. That’s why we’ve released a major update to Qmax designed to align the platform more closely with the 2026 USMLE exam interface and testing workflow.

These updates are now live across Qmax Timed and Untimed tests, giving students a more realistic practice environment and more control over how they take exams.

A Redesigned Test Interface

The Qmax test experience now reflects the updated layout students will encounter on exam day. The interface has been rebuilt to feel cleaner, faster, and more in line with the NBME testing environment.

One of the biggest additions is the new Settings panel, accessible directly from the test header. Students can now customize their testing experience in real time with options including:

  • Adjustable text sizing (100%, 125%, 150%, or 200%)
  • Multiple color themes, including a dark mode option
  • Answer choice strikethrough functionality for eliminating options during questions

These tools are designed to help students tailor the interface to their own comfort and accessibility preferences while staying as close as possible to the real testing experience.

New Built-In Image Tools

Medical board prep relies heavily on visual interpretation, so image handling has been significantly improved.

Embedded images now include integrated controls directly inside the test window. Students can:

  • Zoom in up to 4×
  • Adjust image contrast
  • Invert brightness for improved visibility

This eliminates the need to open separate image viewers and creates a smoother testing workflow during practice sessions.

Faster Navigation During Question Sets

Sequential question sets now include a dedicated “Proceed to Next Item” button that appears after answering a question, helping students move efficiently through blocks without unnecessary scrolling or interruptions.

We’ve also introduced inactivity warnings that simulate the USMLE’s unauthorized break behavior. This helps students build better awareness around timing, pacing, and test-day endurance.

Additionally, Lab Values and Calculator tools remain persistently accessible throughout exams and automatically reposition themselves to avoid blocking one another.

Updated Simulation Structure for 2026

Qmax simulations and self-assessments now follow the latest NBME exam structure for both Step 1 and Step 2 CK.

Step 1 Simulations

  • Full-Test Simulation: 14 blocks, 7 hours, 280 questions
  • Half-Test Simulation: 7 blocks, 3.5 hours, 140 questions

Step 2 CK Simulations

  • Full-Test Simulation: 16 blocks, 8 hours, 320 questions
  • Half-Test Simulation: 8 blocks, 4 hours, 160 questions

All Self-Assessment Exams (SAEs) now use the updated half-test structure as well.

If you previously created or started a simulation before this update, your existing exams and results remain unchanged.

A More Realistic Practice Environment

These changes are part of our ongoing effort to make Qmax not only a high-quality learning platform, but also the most authentic USMLE practice environment possible.

The closer your prep matches the real exam experience, the more confident and efficient you can become on test day.

One quick note: font size and dark mode preferences currently reset between test sessions, so students will need to reselect those settings when starting a new exam.
